There is no reason for pessimism and despair
It is difficult to write and talk about congenital defects of the genitals in men. Not accepted. Men always want to be men and, naturally, will not spread about shortcomings in the most intimate sphere of their body. And if a child with a developmental defect was not promptly reconstructed. a teenager or a young man will keep his fatal secret in himself until male nature or an attentive doctor of the teenage office speaks authoritatively at the enlistment or recruiting office, not satisfied with the young man’s usual answer: “I have everything in order,” he won’t ask him to undress.
And what kind of youth would say right away that he’s not all right there? And he lives like this until he becomes quite grown-up, when a sexual question arises before him, which for some reason is embarrassingly silent. As if it doesn’t happen at all. But he is. In the situations that we are going to talk about today, he becomes so acute that he sometimes throws the sick under a train or pushes life calculations in another way.
After all, people do not always know about the possibilities of reconstructive surgery, and the prospect of being forever without a family, never being a full-fledged man is really depressing.
Other birth defects are lucky. Newspapers and magazines write about the correction of congenital heart diseases, take it off; Dozens of films, fame accompanies cardiac surgeons. With birth defects of the face, everything is clear too: immediately correct, no one will hide them, and both doctors and patients know all this. And issues related to sexual life still remain “behind a closed door.” We either fear them, or underestimate them, or are shy … But the statistics are inexorable. The frequency of birth defects of the penis that violate, and often completely eliminate sexual life is great. For every thousand born boys, there are at least 4 to 5 with defects in the penis. (I’m not talking about relatively “harmless” defects of the priest’s organs, such as not lowering the testicles, narrow foreskin, various cysts – they are significant more.) Parents of such children, and then young men themselves should know that modern urology has sufficiently reliable methods for correcting birth defects, so there should be no place for hopeless pessimism and despair.
The most common defects of the genitals in men are hypospadias and epispadias. I will briefly talk about their signs and manifestations so that parents know when it is urgent to show the child to a surgeon or urologist.
HYPOSPADIA – splitting of the lower wall of the urethra (urethra) and curvature of the penis. The degree of splitting of the urethra can be different – from the head of the penis to the perineum. Accordingly, the external opening of the urethra can open on the lower surface of the head, on the penis, scrotum and perineum (hypospadias of the head, penis, scrotal and perineal). The abnormal location of the external opening of the urethra disrupts urination, causes many inconveniences, boys are sometimes forced to urinate “according to the female type” – squatting. With hypospadias, the outer opening of the urethra is often narrowed. which makes urination very difficult and can cause severe kidney and bladder diseases.
The manifestation of hypospadias is the curvature of the penis down. The degree of curvature also varies. For example, only the head may be curved, which, although it violates, but does not exclude, sexual activity. In severe forms of hypospadias (scrotal and perineal), curvature, and sometimes underdevelopment of the penis, makes sex impossible. Naturally, in children, the main complaints are related to impaired urination. A boy cannot urinate like his peers. In young men and adults, complaints about the violation or impossibility of sexuality come to the fore. Both that, and another heavily affects the neuropsychic state, causing depression, an inferiority complex.
The only effective way to get rid of the defect is reconstructive surgery, which is carried out in several stages.
Parents should know: with a sharp narrowing of the external opening of the urethra, the operation should be done as soon as possible. It is advisable to complete both the straightening of the penis and the restoration of the urethra before the child enters school to avoid psychological trauma.
In parallel, such operations are also being done. as the reduction of the testicles (cryptorchidism is often accompanied by hypospadias), the elimination of hermaphroditic primordia, the removal of the mammary glands (with hypospadias they are enlarged).
These operations restore urination and sexual function.
EPISPADIA – splitting of the upper wall of the urethra is found many times less often than hypospadias. There are epispadias of the glans penis, as well as total epispadias.
